A straight metal barbell with a bead at each end. The surface is polished and one of the beads unscrews by hand.
The bead screws on and off
One bead is held on a screw thread. Unscrew that bead, pass the bar through the hole, then screw it back on. Once it is closed, wiggle it with your fingers to check that it does not come loose. Left loose, it will fall out while you move around.

Wash your hands and wipe the jewelry down with rubbing alcohol.

Unscrew one of the beads and take it off.

Screw the bead back on by hand, then wiggle it to check that it does not come loose.

Cleaning and storage
Wipe it with rubbing alcohol before you put it in and after you take it out. Grime builds up inside the threads once the bead is unscrewed, so clean that part as well. Dry it completely and store it on its own so it does not knock against other metal. A scratched surface will scrape the skin, so do not use a piece that has been scratched.
