A curved barbell for a Prince Albert piercing. A Prince Albert is a piercing made from the urethral opening through to underneath the frenulum, and this piece is bent to follow that passage.
Where the two balls sit
The bar is curved rather than straight, so once it is fitted one ball sits at the urethral opening and the other underneath the frenulum. How to use

Before fitting, wash the bar and the balls with soap, wipe them with rubbing alcohol, and wash your hands too.

Unscrew one ball only and leave the other one on the bar.

Apply lubricant to the bar and to the piercing.

Match the direction of the curve to the line of your body, feed the bar in at the urethral opening and bring it out through the hole under the frenulum.

Screw the ball back on and shake it to check that it does not come loose.
Cleaning and storage
After taking it out, wash it with mild soap and water and rinse out anything caught in the threads. It is jewelry that passes through the urethral opening, so wiping it with rubbing alcohol is the safer course. Moisture left behind makes the thread stiff, so wipe it with a dry cloth and let it dry completely. While you are wearing it, rinse around the piercing with saline once or twice a day, and do not share it with anyone else.
